Nonfood-grade bags used in direct contact with food. Observed cooked beef in plastic to-go containers stored in chest freezer by three compartment sink.
Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee with gloves on handle money from guest then return to food preparation without changing gloves. Inspector educated employee proper glove changing procedures.
Toxic substance/chemical improperly stored. Observed spray bottle of Windex stored on cutting board on flip top cooler. Person in charge removed spray bottle.
Handwash sink not accessible for employee use at all times. Observed hand wash sink behind standing reach in cooler and with faucet turned against the wall with food containers stored in front. Faucet unable to turn back into sink without moving containers and reach in cooler blocks direct access.
Manager or person in charge lacking proof of food manager certification. Observed person in charge unable to provide proof of food manager certification. Spoke to operator on phone who was on vacation and has certification in their car.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink next to three compartment sink. Person in charge provided paper towels.
Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed empty fresh water source for hand wash sink. Person in charge acquired fresh water from nearby approved source using food grade hose.
Cutting board has cut marks and is no longer cleanable. Observed cutting board with grooves and no longer smooth and easily cleanable stored on flip top cooler.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee cell phone stored on cutting board on flip top cooler. Person in charge removed cellphone.
Mobile food dispensing vehicle is not self-propelled or otherwise movable from place to place. Observed; interlocking chains around tires of truck. Person in charge unable to provide when last time truck was moved. Cement blocks used to prop truck up.
Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. Observed no visible license number on side of truck.
Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloth used for cleaning food contact surfaces under flat top grill and not stored in sanitizing solution between uses.

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. On shelf below flat top grill, build up of grease.

Shell eggs in use or stored with cracks or broken shells. See stop sale. One shell egg cracked in egg cracked. Operator discarded cracked egg.
Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. On counter next to serving window, shell eggs stored at room temperature (71F). Operator placed shell eggs back in refrigerator. - ambient temperature 40F.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Operator did not have clean up plan. Provided operator with Form 5030-104.
Handwash sink used for purposes other than handwashing. Hand wash station by triple sink was filled with dirty dishes. Operator removed dirty dishes from sink.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Operator stated they have chemical test strips but could not locate.
No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Employee did not have proof of any training.

Employee drinking from an unenclosed beverage container in a food preparation/clean dishware area and the container does not have a dispensing mechanism to prevent hand contact with lip contact area of drink container. Employee seen drinking from coffee mug with no lid.
Food stored on floor. Large containers of mayo were placed on floor next to door of food truck. Operator removed from floor.
