Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cut lettuce (54f - Cold Holding); cheese shredded (47F - Cold Holding) Operator stated that the lettuce from yesterday and she cut it fresh every day and this is supposed to be trashed they didn't open yet,also stated that the shredded cheese was out for prep less than 2 hours ago and she moved it to the reach-in cooler.
No paper towels or mechanical hand drying device provided at handwash sink. Operator provided some.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Provided to operator and they sign it while conducting my inspection.
