Cutting board has cut marks and is no longer cleanable. Observed white cutting board with cut marks on top 3 compartment sink. Manager discarded during inspection.

Opened employee beverage container in a cold holding unit with food to be served to customers. Observed employee personal bottles of water inside reach in cooler. Manager removed and stored in designated area.

Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. As per operator serves raw tilapia in ceviche. At the time of the inspection non present, advised operator to obtain letter of parasite destruction from supplier of raw tilapia before serving same in the ceviche. Non-exempt fish may not be served raw or undercooked. Consumer Advisory must be in place.
Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Observed raw shell eggs stored above precooked chorizo located in the standing reach in cooler.
Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed for the ceviche.
