Records/documents for required employee training appear to be falsified. Employee proof of food handler training for Alex appears the date has been altered. Original in black ink, blue ink used on the ending year on date of training and expiration.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. On cook line in pot on stove, Cook chicken temped at 125F. Operator stated it just finished cooking 20 mins prior he just didn't have a change to put it in the steam table yet. Operator turned stove back on to heat chicken back to 165F.
Handwash sink used for purposes other than handwashing. Handwash sink right when you enter the food truck observed with plastic container in it. Operator moved container from sink.
Food stored outside. In white cooler stored outside of truck on table; a bag of cheese, raw onions, and lemons.
Garbage can located outside has no lid or lid open/broken. Outside trash can observed with no lid.
