Chlorine sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Triple Sink (Chlorine 10ppm), operator adjusted the chlorine level to 50ppm.

Dead roaches on premises.there is one dead cockroach on top of the dishwasher. Operator removed the roach and sanitizer the top of the dishwasher.
Floor area(s) covered with standing water. There us standing water on the floor under and around the ice machine.
Water draining onto floor surface. There is water draining onto the floor from the ice machine.

Nonfood-grade containers used for food storage - direct contact with food. In the make table cooler cooked shrimp, raw sausage and tomato salsa are in non food grade plastic containers. There are multiple foods in non food grade plastic containers in the walk in cooler.

Food-contact surface soiled with food debris, mold-like substance or slime. There is a black mold like substance neat the top of the back and side walls inside the ice machine. There is a black substance on the can opener blade.

Food stored on floor. There is a case of mayonnaise and bottled water stored on the floor in the dry storage area.
