One employee present hired for approximately 2 weeks, no proof of employee responsibilities. Emailed operator DBPR form 5030-103. Owner explained to employee and had form signed.
Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. In walk-in cooler, cooked chicken prepared 4 days prior not dated. Operator placed date on containers.
