Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Open bottle of water stored in shelf with sauces, located over prep table for napkins and a blender. Operator removed bottle of water during time of inspection.

Mobile food dispensing vehicle connected to utilities on more than a temporary basis. Electricity and water connection not temporary.
Mobile food dispensing vehicle is not self-propelled or otherwise movable from place to place. A wooden platform has been added to the rear of MFDV. Operator added a Reach in chest freezer with ice cubes for customers, stored outside on wooden platform. Sandwich press has been removed and replaced with a flat top grill.
Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle.

Unwashed fruits/vegetables stored with ready-to-eat food. Unwashed strawberries stored on top of gallon of milk in Reach in cooler. Operator relocated strawberries during time of inspection.
Wall soiled with accumulated grease, food debris, and/or dust. Wall soiled in front of fryers.
Working containers of food removed from original container not identified by common name. Several squeeze bottles of sauces not labeled. Stored in shelf above hand washing sink.
