Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed Rust stains on side wall of ice bin.

Male employee using cellphone then started to put cooked chicken tenders with tongs in hot hold unit.touched clean togs without washing hands. Explained cellphone is the dirtiest thing and he washed hands.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Missing on one side of the splitter at mop sink. Explained and showed picture of back-flow preventer on google.
