Observed 2 boiler units at 650,000 BTU's without certification located at boiler room/ dry storage. Hotels and Restaurants DDM Chi, informed the Division of State Fire Marshal, the authorized jurisdiction over certification of boilers on 8/23/2023, forwarding the agency information regarding boilers cited at Shoma Bazaar - food hall, 9420 NW 41 St. Doral, Fl. 33178.
A return visit. The problems from the previous inspection had been dealt with.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dish machine located at shared kitchen area, not sanitizing properly, tested at 0ppm. Establishment has a three compartment sink using quaternary sanitizer, advised to sanitize dishware at three compartment sink until dish machine is sanitizing properly.
Time/temperature control for safety food reheated for hot holding not reaching 165 degrees Fahrenheit for 15 seconds within 2 hours. Observed scallop butter (105F - Hot Holding) located under heating lamp at kitchen area, less than 4 hours. Advised to reheat to 135F.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed employee reporting agreement forms provided but not signed by all employees.
