Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. One employee in kitchen handled his personal phone with gloved hands, then proceeded to handle plastic wrap and a knife without first washing hands and putting on new gloves. The employee washed hands before returning to handle clean equipment.
