Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed Cheese empanadas (125F - Hot Holding); chicken empanadas (128F - Hot Holding). As per staff, 15 minutes. Hot holding equipment temperature was increased.
Employee has not received adequate training related to their assigned duties as evidenced by the inability to answer basic food safety questions. Observed staff member not knowing proper concentration of quaternary at 3CS. Coached staff member on correct procedure.
Handwash sink not accessible for employee use at all times. Observed handwashing sink blocked by carry out boxes at front counter.
Juice packaged in the establishment that has not been specifically processed to prevent, reduce, or eliminate the presence of pathogens not bearing a warning label. Observed raw orange and raw dragon fruit and pineapple juice missing date product was processed and address product was produced. Observed at front display grab and go.

Employee removed cell phone. Observed employee food stored at reaching cooler at rear preparation area not separate from ready to eat items. Employee placed all employee food in a container and separated them.
