Certified Food Manager or person in charge failed to notify the division of employee diagnosed with Norovirus, Hepatitis A, Shigella spp., Shiga toxin-producing Escherichia coli, Typhoid fever (caused by Salmonella Typhi) or Salmonella (nontyphoidal). Had an employee tested positive with Hepatitis A.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed the 3 compartment sink solid.
Handwash sink not accessible for employee use at all times. Observed several items in and around the sink.

In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed scoop inside corn with handle in contact with product. Operator placed handle upwards.
