Raw animal food stored over/not properly separated from ready-to-eat food. In the reach in cooler across from the grill, observed raw salmon and raw white fish over ready to eat potatoes and bell peppers. Operator rearranged for proper storage.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At the bar area, observed feta cheese stuffed olives (64F - Cold Holding). Operator stated foods have been put for two hours. Discussed time as public health control with operator. Operator time marked feta cheese stuffed olives. Provided operator with time as a public health control form.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In the main kitchen at steam table, observed tomato based sauce (112F - Hot Holding). Operator stated foods are reheated, then placed on the steam table. Operator turned up the heat.

Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Provided operator with a copy of cleanup of vomit and diarrheal events.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Unable to provide signed forms at time of inspection. Employees began to sign form.
Bowl or other container with no handle used to dispense food. Being used to scoop rice and flour. Operator switched to scoops with handles.
