Operator is doing a special process, conducting reduced oxygen packaging, cooking time/temperature control for safety food to a time and temperature combination lesser than those in the Food Code or operating a molluscan shellfish tank for human consumption without a HACCP plan approved by the Division of Hotels and Restaurants. Observed operator vacuum sealing multiple food items including: cooked beef, cooked chicken, cooked potatoes, cheeses, and raw fish.
Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed only one employee training certificate. Observed at least 4 employees working in the establishment.

Fish not held frozen before, during and after being packaged onsite using a reduced oxygen packaging method. See stop sale. Observed reduced oxygen packaged salmon held for more than 30 days and not frozen throughout the process.

Stop Sale issued due to food not being in a wholesome, sound condition. Observed approximately 10lbs of frozen reduced oxygen packaged salmon that was held for more than 30 days and not frozen throughout the process. Observed no approved HAACCP plan. Operator voluntarily discarded food.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no signed agreements. Emailed the operator the agreements and had the operator sign one.

Equipment drain line draining into handwash sink. Observed ice machine drain line going to the hand wash sink. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed a build up of dust on the middle ceiling vents above the bakers table.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee phone on the kitchen prep table. Operator removed phone.
Employee with no hair restraint while engaging in food preparation. Observed employee handling food without a hair restraint. Operator had him put on a hair net.
Food stored on floor. Observed soy sauce on the floor of the dry storage area. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.

No handwashing sign provided at a hand sink used by food employees. Observed no handwash sign at the female employee restroom. As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee.
Unclean building components, attachments or fixtures. Observed dust build up on the walk in cooler fan covers.
