Observed quiche pulled from oven at 1120am found at quiche (76F - Cooling) at 220 pm. Allowing 1 hour from cook temp to 135F. Correction by putting items in freezer for 15 min to quickly cool below 70F.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Deli turkey (53F - Cold Holding); Deli ham (51F - Cold Holding); Chicken (53F - Cold Holding); Steak filet (53F - Cold Holding) Operator stated has been in prep table and reach in cooler an hour, operator put the food on ice to bring down to correct temperature and put the food in the reach in cooler in the walk in cooler to bring down he temperature.
Soap dispenser at handwash sink not working/unable to dispense soap. Observed could not dispense soap at the hand wash sink in the kitchen.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed a black and pink like substance on the guard in the ice machine, employee cleaned and sanitized.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Sanitizer reading of the dishwasher 00ppm chlorine. Manager stoped using the dishwasher, set up the three compartment sink to properly sanitize and called the technician.
Handwash sink not accessible for employee use at all times. Items stored in the hand washing sink in the dishwasher area and next to the cook line. Chef removed the items.

No drainboards or equivalent provided for soiled items and/or air drying cleaned items. Observed no space for air drying the amount of dishes that the establishment uses. Manager states that they dry dishes with a towel.. all dishes must be placed in a way after washing that air circulation dries the dishes before stacking or placing for use.
Single-service articles stored next to handwash/food preparation sink exposed to splash. Observed large box of plastic wrap stored under paper towel dispenser at kitchen.
Stored food not covered. Observed open bread in freezer opened and not recovered.

Observed cooked onions (61F - Cold Holding); cooked mushrooms (63F - Cold Holding); bacon jam (63F - Cold Holding); cooked peppers (65F - Cold Holding) stored on counter during rush period. Manager stated not on counter more than 1 hour and replaced in cooler to quickly recook to 41F within 4 hours.
Dog permitted in outdoor dining area without local ordinance allowing pet dogs in outdoor dining areas. Operator states it is a service animal per guest.
Employee with no beard guard/restraint while engaging in food preparation. Observed cooks on cookline with no beard guard. Manager had employees add. And wash hands.
