Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed cooked chicken (51F - Cooling) in the walk in cooler. The operator stated the chicken was prepared the day prior. The operator discarded the chicken.
Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed the dishmachine chlorine solution at 0ppm after being ran three times.
Pesticide/insecticide labeled for household use only present in establishment near the ice machine.
Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw chicken stored over ready to eat sushi ginger in the walk in cooler.

Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cooked chicken (51F - Cooling) in the walk in cooler. The operator stated the chicken was prepared the day prior. The operator discarded the chicken.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed the soda nozzles at the soda machine soiled with a slime-like substance. The operator removed the nozzles for sanitization.
Handwash sink not accessible for employee use at all times. Observed a tray of mussels thawing in the hand sink near the front entrance. The operator moved the mussels to the walk in cooler.
Handwash sink used for purposes other than handwashing. Observed ice dumped in the hand sink at the wait station.

Unused, unprotected place settings left on table with seated customers reused for other customers without cleaning them first. Observed employee putting a cutting board standing up on floor in between prep able and storage rack after cutting vegetables, then putting back on prep table to use for chicken. Made employee washing, rinse and sanitize cutting board before using.
Wet mop not stored in a manner to allow the mop to dry. Observed mop heads sitting on floor wet, not hung up or inverted to dry properly.
