Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In ice bath on prep table by reach in freezer, sausage patty (44F - Cold Holding). Manager stated they have been out since 6 am. Manager put more ice in ice bath to bring temperature down. Took temperature 20 minutes later at 41F.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine soiled with slime/mold-like substance.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling vent about server window soiled with dust.
Cove molding at floor/wall juncture broken/missing. Cove molding under drive through window missing. Manager has been trying to get in contact with company that is suppose to fix wall/cove molding.

Toxic substance/chemical improperly stored. In kitchen, spray bottle of Lysol on prep table. Manager moved bottle to correct place.
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Provided manager with written procedures for Clean-Up of Vomiting and Diarrheal Events.
No proof of required state approved employee training provided for employee hired more than 60 days ago. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. One employee hired December 15th does not have training. Manager stated he will be doing it this week coming up.

Both employees on duty do not have health reporting paperwork filled out. Manager read over health reporting agreement with both employees and had them sign.
