Observed food debris buildup on floor, especially under equipment, along far wall where back door meets floor.

Employee stated they didn't have space in cooler for the foods. Employee stated items have been out for about an hour. Employee placed items on ice.

Observed fan at top was blocked by multiple pans of food preventing cold air to flow to foods below on other shelves. Operator rearranged cooler to chill cheese and pork.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash sink near truck entry door. Operator replenished paper towels.
No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Operator unable to provide proof of employee training. Operator stated training material has been ordered.

No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No test strips for chlorine sanitizer. Operator stated they got wet when dropped into solution when making sanitary solution.
