Cardboard used to line food-contact shelves. Soiled cardboard lining food contact shelf in hot holding unit. No food in unit. Operator removed cardboard.
Dead roaches on premises. 1 dead roach under hand wash sink. 1 dead roach on floor by entrance 1 dead roach in bathroom. Operator cleaned ad sanitized all areas.
Exterior door has a gap at the threshold that opens to the outside. Back door has light coming in from all sides.
