Observed in their make table and grill reach in cooler: Chicken (53F - Cold Holding); Beef (53F - Cold Holding); Pork (53F - Cold Holding); Chopped tomatoes (56F - Cold Holding); Sliced cheese (56F - Cold Holding). Operator stated that the product had been there for approximately 3hrs. Operator placed them in their walk in cooler with ice.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Testing kit revealed 0ppm for 3 cycles at dish surface. Operator advised to set up 3 compartment sink for sanitizing. Operator states they will contact vendor for service.
Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee cooking food handled raw chicken and beef then proceeded to handle clean dishes for service. Operator advised to stop process and clean and sanitize dishes, remove gloves and wash hands.
Nonfood-grade bags used in direct contact with food. Found in reach in freezer, containing raw frozen chicken, non food grade bag. Found in reach in cooler, containing raw pork trimmings, in non food grade bag. Advised operator to only use food grade bags for storage. Operator removed product and discarded on own recognizance.
Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Dish washer handle dirty dishes then handled clean dishes, no glove change or hand washing.
Food-contact surface soiled with food debris, mold-like substance or slime. Multiple cutting boards in kitchen visibly soiled and not clean. Operator advised of need for cleaning and sanitizing food contact surfaces.
Handwash sink used for purposes other than handwashing. Observed kitchen employees using hand wash sink to fill container with water for supply on cook line. Operator advised to use designated hand wash sink for handwashing only.
No paper towels or mechanical hand drying device provided at handwash sink. Hand wash sink in kitchen and at front counter, no paper towels. Operator replaced with paper towels.
No probe thermometer provided to measure temperature of food products. Operator advised to obtain and utilize thermometer frequently.
Accumulation of debris on exterior of warewashing machine. Operator advised of need to clean exterior of dish machine.
Accumulation of lime scale on the inside of the dishmachine.

Operator discarded cooked chicken, cooked pork, and Flan; remaining items less than 4 hours in reach in cooler. Operator in process of moving all items to another cooler for quick cooling.
