Spray bottle containing toxic substance not labeled. At the dish washer station, a spray bottle of bleach water was stored on the faucet. The bottle was emptied and removed. At the bar, a spray bottle of general lavender cleaner was not labeled. The chef labeled the bottle.
Food stored on floor. In the walk in cooler, a bucket of peeled potatoes were stored on the floor. The bucket was moved up off of the floor.

Dishwasher (Chlorine 0ppm). The dishwasher corrected the hose and the dishwasher was retested at 100ppm Chlorine

Premium reach in cooler; American cheese (46F - Cold Holding); mozzarella cheese (45F - Cold Holding) in premium reach in cooler at the left end of cook line. Observed Ice build up behind cheeses in reach in cooler. Cheeses held in unit under 4 hours ago, operator moved to walk in cooler to quick chill. Foods not portioned or prepared today.
Food-contact surface soiled with food debris, mold-like substance or slime. 1. Cutting board across from grill soiled in cook line. 2. Soda nozzle soiled at bar. Operator cleaned and sanitized during inspection.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. One new employee( Matt) hired less than 60 days, operator unable to provide employee reporting agreement. Per manager employee file is with the accountant.
