Observed the shelving used to store clean dishware above the hand sink near the grill soiled with an accumulation of dust and debris.

Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw ground beef stored over squeeze bottles of 1,000 island dressing and horseradish sauce in the reach in cooler near the grill. The raw ground beef was properly stored.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed deli ham (58F - Cold Holding) stored stacked above the fill line in the top section of the reach in cooler at the service window. An employee moved the ham to the lower portion of the reach in cooler. Observed raw ground beef (56F - Cold Holding); cooked rice (54F - Cold Holding); cooked sliced beef (57F - Cold Holding) stored stacked above the fill line in the top section of the reach in cooler near the walk in cooler. An employee moved the ground beef to the walk in cooler. The employee emptied half off the contents of the sliced beef and cooked rice and moved the foods to the walk in cooler. Observed sliced cheese (53F - Cold Holding); cooked chicken (52F - Cold Holding) stored stacked above the fill line in the top section of the reach in cooler near the grill. An employee emptied half of the contents and moved the food to the walk in cooler. Observed raw chicken (46F - Cold Holding); raw tilapia (47F - Cold Holding) in the breading station. An employee placed bags of ice on the raw foods. Employees stated the potentially hazardous foods had been held within 1 to 2 hours.
No paper towels or mechanical hand drying device provided at handwash sink near the ice machine. The operator provided paper towels for the hand sink.
