Hidden raw/undercooked ingredient with no reminder such as an asterisk to alert the consumer advisory applies. Observed smoked salmon offered on menu with no asterisk. Operator will add asterisk to menus.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sliced cheese (50F - Cold Holding) stored stacked above the fill line in the top section of the reach in cooler in the kitchen. The operator stated the cheese had been held for approximately 1 hours. The operator emptied half of the contents and stored in the lower half of the reach in cooler.
Toxic substance/chemical improperly stored. Observed a spray sanitizer bottle stored near condiment bottles near the entrance to the kitchen. The operator properly stored the spray bottle.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed no quaternary test kit.
Food stored on floor. Observed plastic bottled water stored on the floor in the kitchen. The operator properly stored the water.
Water leaking from pipe and/or faucet/handle. Observed water leaking from the quaternary solution dispenser at the three compartment sink.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed dish machine only reaching 147F. Operator called technician and set up manual sanitizer.
Operator is doing a special process, conducting reduced oxygen packaging, cooking time/temperature control for safety food to a time and temperature combination lesser than those in the Food Code or operating a molluscan shellfish tank for human consumption without a HACCP plan approved by the Division of Hotels and Restaurants. Observed vacuum sealing of cooked product being conducted with no HACCP plan. Operator has variance for vacuum sealing fish in 10K bags, but no other HACCP plans.
