Toxic substance/chemical improperly stored. On metal rack next to three compartment sink boxes of hand soap and hand sanitizer stored above clean pan lids and utensils.
Handwash sink not accessible for employee use at all times. Handwash sink in back kitchen has fruit/vegetable wash bucket in sink. Employee removed bucket from handwash sink.
Food stored on floor. Box of chips on floor in back kitchen area.

Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of bread toaster at front counter soiled with food debris build up.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple gaskets at front counter soiled with food debris. Also, fan covers in walk-in cooler soiled with dust build up. Also, on exterior of ice chute and around nozzle on soda machine in lobby soiled with soda residue/mold-like substance. Also, ceiling vents at front counter soiled with dust build up.
Opened employee beverage container in a cold holding unit with food to be served to customers. In stand up reach in freezer and reach in cooler in back kitchen employee drinks on shelves with customer food. Operator moved all drinks and stored correctly.
Reach-in cooler interior/shelves have accumulation of soil residues. Interior of reach in cooler at front counter soiled with food debris.
Unwashed fruits/vegetables stored with ready-to-eat food. In walk-in cooler, unwashed tomatoes and onions stored above open boxes of ready to eat meats.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer bucket at front counter, Sanitizer Bucket (Quaternary 0ppm). Operator made new sanitizer bucket and retested at 200ppm.
Met inspection standards. 12 violations recorded, none serious enough to require a return visit.

Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Sanitizer tested 0ppm at triple sink. Solution discarded. Solution remade manually - tested 200ppm.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In flip top cooler on sandwich make line, pan of sandwich meat set ups 62F. Pan is placed on top of other pans and is not exposed to refrigeration. Pan placed on top approximately two (2) hours prior. Pan of meats moved to reach in cooler to cool.
