Stop Sale issued on time/temperature control for safety food due to temperature abuse. Cut tomato in bottom of small reach in cooler at 45F. Per manager, tomatoes in same unit overnight.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. See stop sale. Cut tomatoes in top of small cold bar at 47F and cut lettuce at 45F. Per manager, both placed in cooler approximately 30 minutes prior. Manager voluntarily discarded lettuce and placed tomatoes in reach-in freezer to quickly chill and approximately 10 minutes later temperature is 38F. Also, cut tomato in bottom of small reach in cooler at 45F. Per manager, tomatoes in same unit overnight.
No proof of required state approved employee training provided for employee hired more than 60 days ago. Per manager, missing seven employee's proof of food handler training.

Observed cut tomato (47F/46F/49F - Cold Holding); cut lettuce (47F - Cold Holding) in small flip top reach in cooler. Per manager, all products have been in the same cooler overnight. Inspector observed ambient thermometer at 45F.
Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Paper towel dispenser in men's restroom would not dispense paper towels. Employee replaced batteries and dispenser is now working.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Vents above sandwich make line have dust.
Food stored on floor. Pickle bucket on the floor in front of three compartment sink.
Plumbing system in disrepair. Cold water valve on handwash sink at prep area does not work. Also, handwash sink on make line is slow draining.
Met inspection standards. 1 violations recorded, none serious enough to require a return visit.

Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Cook preparing sandwiches with bracelet on right wrist. Employee removed bracelet.
