Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed interior of ice bin soiled. Operator started cleaning it during this inspection.
Equipment in poor repair. Observed torn gasket on reach-in cooler across from POS station.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Observed Sanitizer Bucket (Quaternary 0ppm) at front service line. Operator emptied and remade solution and retested at 150ppm.

Also, employee sweeping floor, then put on gloves without washing hands first. Operator discussed hand washing procedures with employees

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cut lettuce (50F - Cold Holding)in top of flip top reach-in cooler at sandwich making station. Observed bag of cut lettuce stored above chill line of reach-in cooler. Employee placed bag of cut lettuce in bottom of reach-in cooler to chill. Also, house made tuna (46F - Cold Holding) in top of flip top reach-in cooler at sandwich making station. Employee stated the lid was opened during lunch. Employee closed lid to chill tuna.
