Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In the steam well of the cook line, chili (124F - Hot Holding); au jus (114F - Hot Holding); cooked onions (117F - Hot Holding); soup (121F - Hot Holding) steam well water very low. Operator added water and began reheating all products in the microwave. At conclusion of inspection all products reheated over 165 and placed back in steam well.
Food-contact surface soiled with food debris, mold-like substance or slime. Burned on food debris on conveyor belt of bread toaster.

In the steam table closest to the end of the cook line by the wall, loaded potato soup (91F - Hot Holding) operator placed soup in the microwave to reheat to proper temperature. At conclusion of inspection loaded potato soup second temperature (156F - Hot Holding).
No soap provided at handwash sink. No soap at hand wash sink closest to the walk in cooler, per operator switched chemical provider and soap not replaced in dispenser.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Bulk ice bin has mold like substance on the deflector plate.

Ice scoop handle in contact with ice. Drive through soda machine ice scoop stored in direct contact with drink ice. Operator removed scoop and coached employee on storage.

Observed employee pick up items off of the floor, and pull up pants with gloved hands, remove gloves, put on new gloves and handle cheese without hand wash. Spoke to employee during inspection.
Establishment operating with changed menu and/or increased seating without septic system approval for such change. Establishment is licensed for 32 seats, has 30 inside, 12 outside. Is on municipal water and sewer. Observed signature from Fire Jurisdiction.

Person in charge states were heated in microwave prior to placing on steam table. Will reheat in microwave as they have been out of temperature for less than 4 hours.
