Opened employee beverage container in a cold holding unit with food to be served to customers. In prep area reach-in cooler, 2 employee drinks with lids and straws stored on upper shelves with customers foods. Operator removed drinks.
Unwashed fruits/vegetables stored with ready-to-eat food. In prep area single door reach-in cooler, case of unwashed peppers stored over container of prepped onions. Operator moved onions.

Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. At front counter by registers, cookies and brownies packaged onsite missing labeling. Operator stated customer picks up item.

Ceiling/ceiling tile shows damage or is in disrepair. Ceiling in front of walk-in cooler, ceiling tiles missing. Operator stated contractor was doing wiring work and broke ceiling grid. Contractor is in process of repairing ceiling.
Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Sanitizer bucket stored on prep table, operator moved to lower shelf.
