Observed live roaches in the following locations one live roach on microwave, four on trash can near microwave and toaster oven, ten on box under counter near microwave toaster oven, one live on prep table where cookies are stored, three on sleeve of sandwich containers, two in receipt box under cash register, one near ice machine, one on 3 compartment sink,six near whole meat reach in cooler, one near reach in cooler across from 3 compartment sink.

Observed dead roaches in the following locations three dead roaches near handwashing sink located by meatballs, one on the wall by same handwashing sink, twelve up under cash register, one near ice machine, two near mop sink, approximately eighty on trap up under storage rack in rear of kitchen.

Soiled dry wiping cloth in use. Observed dry wiping cloths soiled on prep tables.
Water leaking from pipe and/or faucet/handle. Observed faucet leaking at handwashing sink near ice machine.
