Observed employee, while wearing gloves, touch pants and wipe hands on apron, then continued prepping food order without washing hands and changing gloves. Educated employee on proper hand washing and food handling procedures. Employee washed hands.
Handwash sink not accessible for employee use at all times. Observed measuring pitcher stored inside hand wash sink at front counter.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no food employee agreement form signed for new hire currently working. Reviewed agreement form with employee and she signed during inspection.

Per operator, all products Reheated in microwave prior to being placed in steam well. Per operator, out of temperature for approximately 15 minutes. Operator returned all products to microwave to reheat to 165+F.
