Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee swept the floor, changed the trash and didn't remove the gloves before engaging in food preparation. I informed the operator and he had the employee to remove the gloves and wash his hands and put on a new pair of gloves.
