Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed sliced cheese (52F - Cold Holding); tuna salad (48F - Cold Holding); cooked chicken (47F - Cold Holding); cooked roast beef (48F - Cold Holding); cooked ham (46F - Cold Holding); cooked turkey (47F - Cold Holding) in the make line #1 cold holding unit. A temperature log stated an ambient temperature of 41F was recorded approximately 2.5 hours prior to inspection. The operator will continue to monitor temperatures.
Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloths stored on cutting boards in-between uses. The operator removed the wet cloths.
Marked exit/path to marked exit blocked. For reporting purposes only. Observed the rear exit pathway blocked by discarded cardboard boxes. The operator removed the boxes from the doorway.

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets on reach in, on line, soiled with food debris. Observed employee cleaning gaskets.

Employee with no beard guard/restraint while engaging in food preparation. Observed employee scooping ice, filling ice machine for the soda station without a beard guard.

Soap dispenser at handwash sink not working/unable to dispense soap. Observed at hand sinks throughout establishment, soap dispensers not dispensing.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed inside ice machine build up and black mold like substances at on shield.
Buildup of food debris/soil residue on equipment door handles.Observed on back side of handle of oven by handwashing sink, with food debris on it. Observed oven handle being cleaned.
Cutting board has cut marks and is no longer cleanable. Observed red cutting board above microwave with groove marks through out cutting board.
Equipment in poor repair. Observed on tall stand up reach in coolers throughout kitchen, gaskets peeling off rubber and sticking to door frame in pieces.
In-use utensil not stored on a clean portion of food preparation or cooking equipment. Observed cooking utensils stored in a bus tub with dried food debris on bottom of bus tub. Observed operator remove utensils and had dishwasher, wash all utensils and bus tub.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed food debris inside of microwave on line at front counter. Observed employee cleaning out inside of microwave.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employees changing gloves with out washing hands first. Spoke with GM, she In turned coached the staff.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed food debris on potato chip shelf, above prep table, not cleaned. Observed employee cleaning and sanitizing shelves, after being coached.
Bowl or other container with no handle used to dispense food. Observed in spice containers, Togo soup cups being used for scooping product and left in containers with no handles. Spoke with Operator and she removed containers out of spice containers.
