Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. milk, sweet whipped cream (48F-50F - Cold Holding) in glass door reach-in cooler next to coffee equipment. Operator stated items placed in cooler this morning. Observed ice build up around fan area. Operator removed TCS foods from cooler and placed in nearby cooler to chill. Also, raw salmon (52F - Cold Holding) in top of flip top reach-in cooler across from egg station. Operator stated items placed was placed in cooler this morning. Operator placed a bag of ice in salmon and placed in bottom of cooler to chill. Retemped items at end of inspection and all at 41F and below.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed ice deflector plate in large ice machine soiled with black debris. Operator had employee start to clean during this inspection. Also, portable white cutting board on cook line soiled/ stained.
Handwash sink used for purposes other than handwashing. Observed employee using hand wash sink at front service line to rinse out wiping cloth. Operator discussed with employee that hand wash sink is for hand washing only.

Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed glass door reach-in cooler on front service line near coffee equipment unable to maintain temperature of 41F and below. Observed large mass of ice covering coils of fan in back of interior cooler. Operator removed TCS foods from cooler and placed them in nearby cooler until cooler is serviced.
In-use tongs stored on equipment door handle between uses. Observed pair of tongs hanging from oven handle on cook line. Operator removed tongs to properly store.

Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer Bucket (Quaternary 0ppm); at prep area. Operator remade sanitizer solution and retested at 200ppm.

Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Whipped butters in souffle cups and in pan, cooked potatoes, with inadequate time mark. Establishment ipad used for tracking time shows 9:30 am, 1:30 am. Person In Charge reset iPad and marked butters, potatoes prepped few hours ago.
