Establishment has no approved Alternative Operating Procedure. Observed employee pull toast out of the toaster with her bare hands, observed employee discard toast.
Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tiles above servers station damaged with water damage.
Clean equipment/dishware/utensils stored next to handwash/food preparation sink exposed to splash. Observed coffee cups stored next to hand washing sink with paper towels store above them, the operator moved the coffee cups.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee wearing a watch while serving food.
Reach-in cooler shelves with rust that has pitted the surface. Observed rusting on the reach in coolers on the Cook line shelves.
