In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed the scoop handle used for dried granola not stored above the food. The operator properly stored the scoop handle.

Observed employee drop a knife and towel on floor, picked up, and didn't wash hands or change gloves. Observed operator coaching, and correcting employee. Employee washed and changed gloves.

Employee with artificial nails/nail polish working with exposed food without wearing intact gloves. Observed employee expo station with nail polish on, working the expo, without gloves on. Observed operator, coach employee, employee wash, hands, put on gloves.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed a cutting boards on make in prep area with black mold like substance on it. Observed operator throw away cutting board. Observed employee knife onto floor, pick up knife, use sanitize cloth to start cleaning the knife off, put sanitize towel on prep table.
Handwash sink used for purposes other than handwashing. Observed both handwashing sinks in service area being used as dump sinks with brown tea/coffee stains by coffee station. And fork, and paper in drain across from expo station.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed on ice shield in ice machine, some black mold like substance.
Cutting board has cut marks and is no longer cleanable. Observed a cutting boards on make in prep area with grooves on it. Observed operator throw away cutting board.
Equipment and utensils not properly air-dried - wet nesting. Observed employee stack cambros that just came out of the dishwasher, stacked wet nesting. Observed operator, coaching, and correcting employee, and separating cambros.
Equipment in poor repair. Observed torn gasket at egg station, reach in coolers.
Light shield damaged/in disrepair. I'm so broken in light shield to the left of the walk-in cooler, above the pre table.
Silverware/utensils stored upright with the food-contact surface up. Observed a container of silverware, stored with handle stored down. Served the operator the container and stated that those are from back storage and needed to go into the office.
Single-service articles not stored inverted or protected from contamination. Filters and a container not covered. Observed recovery, coffee filters.

Silverware/utensils stored upright with the food-contact surface up. Observed dishwasher bring up a rack of dried silverware to be stored, with handles down on forks and spoons. Observed GM taking basket back to dish for rewatch and proper storing.

Observed employee handling iPad for recipes then proceeding to cook food with out washing hands and changing gloves. Observed Manager on Duty coaching said employee and observed employee washing hands before returning to work station.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels in paper towel dispenser by drink station. Observed Manager on Duty refilling paper towels.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed five third pans stored on dry storage rack up instead of open End of Day down. Observed Manager on Duty correcting by putting containers face down.
Single-service articles improperly stored. Observed single serve Togo containers inverted with open up. Observed Manager on duty correcting immediately by inverting said containers face down.
Working containers of food removed from original container not identified by common name. Observed sugar and grits in containers with no identifying names on them. Observed Manager on Duty labeling said containers.
