Per operator items were not prepared or portioned today. Items have been out of temperature for approximately 45mins. Manager placed items in walk-in freezer to rapid cool. -observed pico (72F - Cold Holding); fruit salad (72F - Cold Holding). Items were out of temperature for approximately 3 hours. Manager voluntarily discarded.

No paper towels or mechanical hand drying device provided at handwash sink. Observed paper towel dispenser not working correctly at hand wash sink next to dishwasher. Operator was able to fix the dispenser.
Food stored on floor. Boxes of pineapples stored on floor in walk-in cooler.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup inside walk-freezer.
