Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee working in cook line touched back of pants, then wear gloves without washing hands. After manager instructed, employee discarded gloves, washed hands and put in new pair of gloves.

Certified Food Manager or person in charge does not require employees to report illnesses and symptoms as required. Advised operator to have new employees review and sign food employee reporting agreement form.
Comminuted meat not reaching a minimum internal temperature of 155 degrees Fahrenheit for 17 seconds or equivalent time/temperature combination as specified in cooking chart. Observed double cheeseburger (112F - Cooking) at grill. Employee put burger back on grill and cooked to 175F.
Employee personal food not properly identified and segregated from food to be served to the public. Observed employee beverage and food stored inside walk in cooler, not separated from restaurant food. Operator removed.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed at cookline grilled onions (98F - Hot Holding). Per operator, food out of temperature for approximately 2.5 hrs. Operator put onions on grill to reheat to 165F for hot holding.
