Toxic substance/chemical improperly stored. Five aerosol cans containing spray paint stored directly on top of bin containing mustard packets and directly beside box containing "cinnamon toast crunch" cereal. Manager removed the paint from the area during this inspection.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Threaded faucet on mop sink behind building missing backflow prevention device.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Employee scooped ice cream while wearing a watch on wrist.
Hole in or other damage to wall. Hole in wall inside kitchen near entrance door.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior of microwave soiled with food debris.
Unclean building components, attachments or fixtures. 1. Two fans attached to walls near handwash sink and near three-compartment sink soiled with dust. 2. Walls inside single-use item storage shed soiled with mold-like substance. 3. Floor along walls throughout kitchen soiled with dirt and/or food debris.
Unnecessary items/unused equipment on the premises. Unused equipment and buckets scattered on ground around mop sink behind building.
Unwrapped single-service utensils not presented so that only the handles are touched. Basket containing single-use spoons not presented with handles facing upwards. Manager moved spoons to another container with handles facing upward during this inspection.

Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Open packages of ham and turkey missing date marks inside reach-in cooler. Manager stated both were opened on 11/18 and added the proper date marks during this inspection.

Chlorine sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Three-compartment sink chlorine 200ppm. Inspector had manager drain half of the sink and refill to dilute the chlorine to 100ppm.
Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Porous acoustic ceiling tiles throughout back prep area of kitchen.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee purse on top of package of bottled waters. Employee removed purse from top of bottles during this inspection.
Grease receptacle/dumpster not on proper pad/nonabsorbent surface. Dumpster sitting directly on grass behind building.
