Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed a hose connected at the time of the inspection by the bathroom at the dry storage and.
No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Observed walking cooler was removed and placed at the dry storage and in that space they installed a rotating rack oven. Operator needs to proved plans to plan review.
Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Observed at bathroom located at the storage area.
Cutting board has cut marks and is no longer cleanable. Observed white cutting board in the kitchen area.
Exterior door has a gap at the threshold that opens to the outside.

Nonfood-grade bags used in direct contact with food. Observed raw chicken in no food-grade bags stored in reach in cooler and reach in freezer.
Ceiling/ceiling tile shows damage or is in disrepair. Observed above convection ovens.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed pots not inverted in storage area near restroom and shelf at entrance.
