Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed employee ware wash dishes at triple sink then began to touch lid on bottle with sour cream; no hand wash. Employee washed hands and empty the contents of bottle into a clean bottle. Observed employee ware wash containers at triple sink, touch forehead then began to handle clean bottle with sour cream to label; no hand wash. Manager advised employee to wash hands and change bottle of sour cream to a clean one.

Observed employee wash several containers and cutting boards with soap and water; no sanitization done. Manager had employee set up sanitization sink to sanitize with quat solution 200ppm
