Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee touched arm/shirt sleeve, donned gloves, and continued food preparation without washing hands. Manager hand employee wash hands and change gloves.
Food-contact surface soiled with food debris, mold-like substance or slime. Prep reach-in cooler cutting board on cookline soiled with mold-like substances.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at cookline hand wash sink. Manager replaced towels.
Equipment in poor repair. Ice buildup in reach-in freezer. Cutting board grooved. Fan guard missing in microwave near ware washing area.

Old labels stuck to food containers after cleaning. Old label/label residue on clean bins on rack across from three compartment sink. Manager removed for cleaning.
Wet mop not stored in a manner to allow the mop to dry. Wet mop in mop basin not hanging to dry. Manager hanged to dry.
Wiping cloth DDBSA sanitizing solution not at proper minimum strength. At hanging sani-bucket across from warewashing area: Sanitizer Bucket (DDBSA - 500ppm); Sanitizer Bucket (DDBSA - 0PPM).

Employee eating in a food preparation or other restricted area. Observed : cook line staff chewing on food as I entered the kitchen. The manager had a discussion with the staff member. There is a designated employee eating area.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed : cut tomatoes 49° on the cook line. These were moved to the walk-in cooler and achieved 43° in 9 minutes. These were cut around 11 am today, and came from the walk-in cooler for stocking after lunch.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed : cheese on the cook line at 118°. The warmer was adjusted, cheese hot 165°.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed the tomato slicer has tomato residue on the pusher head and on the blades. This was sent back to dish.
Handwash sink used for purposes other than handwashing. Observed ice was in the cook line hand wash sink.
Carbon dioxide/helium tanks not adequately secured. Observed : the chain securing the pressurized tanks was pulled from the wall. Additional material was added and tied to the adjoining shelf.
Single-use gloves not stored in a clean, protected location. Observed : opened black single use gloves stored next to sanitizer(700 eco sure). The gloves were moved.
Table service items not handled in a sanitary manner. Observed : staff wrapping forks, and picking them up by the food contact end. Hands were washed and gloves were added.
A return visit. The problems from the previous inspection had been dealt with.

Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. Observed : on the cook line no time mark for the diced tomatos. A time mark was added with 1.75 hours left.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed the bar the soda gun, at the point of the bar, was not wrapped and soiled. **Repeat Violation* ALSO, the tea nozzles at the back wait station have buildup.

Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed: a quaternary amonia sanitizer bucket on the prep line stored with bulk flour on the lower shelf. The bucket was moved away.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed: the dish wash machine could not turn the test strip black after multiple attempts.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed on the cook line: slaw 51, house dressing 57°. Each item was moved uncovered to the walk-in freezer. Items were restocked from the walk-in cooler after 2:30 pm.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed lack of back flow device on the non chemical side of the splitter in the mop sink.

Single-service articles improperly stored. Observed gloves used for food service are stored in the room by the back door. The ceiling in that room is an expanding foam ceiling.
