Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. In reach in freezer raw bacon in open package stored over case of ready to eat muffins. Person in charge moved to appropriate location.
Multiuse food-contact surface not smooth, free of breaks/cracks/chips/pits/crevices. Ice buckets with cracked edges, missing pieces. Person in charge moved to be discarded.
No soap provided at handwash sink. No soap observed at hand wash sink at front cookline. No soap and paper towels observed at bar area. Person in charge provided during inspection.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Underside plate of juice dispenser with debris accumulation. Interior of keg cooler with debris accumulated.
Open condiments provided for self-service not properly protected. Self serve toppings for oatmeal missing hinged lids.

Dishmachine in use testing at 0ppm. Unit is out of sanitizer and person in charge does not have a new one to replace. Person in charge set up triple sink at 200ppm to utilize until unit can be repaired.

Raw bacon no longer commercially packaged stored over ready to eat ham in opened package in reach in freezer. Raw steak stored over oranges in reach in cooler. Person in charge began rearranging.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Mop sink missing at hose Bibb on non chemical side of splitter. Person in charge removed splitter during inspection.
No paper towels or mechanical hand drying device provided at handwash sink. Missing paper towels and hand soap at back hand wash sink.
No proof of required state approved employee training provided for any employees. No proof of employee training due to only employee with expired training.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. liquid eggs (46F - Cold Holding); cut tomatoes (48F - Cold Holding) at counter next to egg burners. Observed pans not touching the ice in big pan. Employee re-iced eggs and tomato's.
Toxic substance/chemical improperly stored. Spray bottle containing toxic substance stored next to container of coffee on shelf below coffee urns. Operator removed spray bottle of sanitizer.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker missing from non chemical side of splitter at mop sink.
