Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed : the dish wash machine at 0 ppm for chlorine. A dispenser cap was removed from the drain. No change. The new gallon jug was already in place. Primed multiple times, no change. The manager will call eco lab tech. Dishes will be sanitized manually.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed : sliced cheddar cheese 59° and sliced jalapeño cheese 57°. These had been in a chill pack but were now on the counter. Cheese moved to a freezer.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed : a large white cutting board on the clean dish rack, stained.
Floors not maintained smooth and durable. Observed : the floor in the walk-in freezer is not solid, it has a raised and lower surface.
