No paper towels or mechanical hand drying device provided at handwash sink. Observed the handwashing sink next to the door with no paper towels, or soap. Employee was able to replace the paper towels.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed an employee's keys on the prep table. Employee removed the keys and stored correctly.
Floor, ceiling tiles missing and/or in disrepair. Observed ceiling tiles with dust and food debris. Observed broken floor tiles in the kitchen next to the handwashing sink. Observed the wall soiled with build up dust.
Food stored on floor. Observed a box of lamb being stored on the floor in the freezer. Employee placed the box 6 inches off the floor.
Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed rusted shelves and unsealed wood in the shelves in the walk in cooler.
