Time/temperature control for safety food removed from cold holding for necessary preparation rose above 41 degrees Fahrenheit during the preparation process and was not cooled back down to 41 degrees Fahrenheit within 4 hours. Raw shrimp was prepped at 11:30am per manager. Temperature at 4:27pm is 51F.
Certified food manager unable to answer basic Food Code questions pertaining to safe operation of establishment. Manager on duty did not know minimum internal cooking temperature for chicken wings.
Non-pitting surface rust on food-contact equipment. Can opener blade has surface rust. Manager states a new one has been ordered.

Stop Sale issued on time/temperature control for safety food due to temperature abuse. Sliced gyro meat in walk in cooler at 52F. Per employee, meat was sliced yesterday.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. See stop sale. Wings in walk in cooler at 44F. Employee states wings were left out of the cooler for approximately 15 minutes. Also, sliced gyro meat in walk in cooler at 52F. Per employee, meat was sliced yesterday.
Vacuum breaker missing/incorrectly installed at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker incorrectly installed at top of added splitter to mop sink. Operator removed splitter and attached hose directly to threaded faucet with atmospheric vacuum breaker present.
No proof provided that some food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. No employee reporting agreement for Nana or RJ both employed less than two weeks. Operator had both employees sign form during inspection.

No sanitizer of any kind available for warewashing. Only use single-service items to serve food to customers until sanitizer is available for warewashing. No sanitizer available. Operator purchased chlorine bleach during inspection.
Carbon dioxide/helium tanks not adequately secured. Two carbon dioxide tanks are unsecured at soda machine at front counter.
No conspicuously located ambient air temperature thermometer in holding unit. No ambient thermometer in two stand up reach in coolers in the kitchen. Operator added thermometers to both units.
