Dead roaches on premises. Observed : dead roaches: 7 behind rice warmer at the wait station 2 behind shelving on the way to the dish wash machine area. 2 by the dish wash machine on the floor 3 by the walk-in cooler door 2 by the back door All were swept up by staff as we moved along.

Required employee training expired for all employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed: all certificates show were expired. 2 updated ones were found.
Equipment in poor repair. Observed: the cook line 2 door reach-in cooler has storm gasket on the bottom edge.
Time/temperature control for safety food thawed under water that was barely/not running/dripping. Observed: frozen chicken in the 3 compartment sink with no water running.
Water leaking from pipe and/or faucet/handle. Observed: the dish wash machine water line under the machine broke during the chlorine test. The manager shut the water off and says he can fix it.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed: the dish wash machine at 0 ppm. This was corrected to 100 ppm.
Operating with an expired Division of Hotels and Restaurants license. Observed the license expired on 6-1-2024. This was corrected on site.
Toxic substance/chemical improperly stored. Observed: a tube of L'Oréal stored over bulk pancake mix, by the back door.

Establishment has no approved Alternative Operating Procedure. Observed: prep cook cutting cucumbers, no gloves. Gloves were added.
Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. Observed: shell eggs at Ambient air temperature of 71F, on the cook line. The owner had the eggs put back in the reach-in cooler. These had been out since 12:15.
