Raw animal food stored over/not properly separated from ready-to-eat food. Raw pork sausage in third walk-in cooler over pasteurized eggs. Spoke with operator regarding pasteurized food storage, operator switched raw pork to bottom shelf.
A return visit. The problems from the previous inspection had been dealt with.

Employee rubbed hands together for less than 10-15 seconds while washing hands. In dishwashing area, observed employee switching from handling dirty to clean dishes with a five second rub during handwash between. Observed this occurring twice. Spoke with manager regarding proper handwashing times.
Heat strip failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface and/or high temperature holding thermometer did not reach 160F. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Dishwasher (Temperature 153F) after running multiple times during use to check temperature. Operator began manual sanitation in triple sink already set up, 300ppm quaternary.
Carbon dioxide/helium tanks not adequately secured. 3 unsecured across from office in hallway by back door.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Dust on vents, left side hallway leaving kitchen.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Vent covers in second and third walk-in coolers.

Unwashed fruits/vegetables stored with ready-to-eat food. In walk-in cooler 2 for vegetables, unwashed asparagus, avocados over ready-to-eat chopped garlic, cut tomatoes. Unwashed heads of lettuce over chopped and washed ready-to-eat lettuce.

Incomplete proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Operator is using a temp service for dishwashers, dishwasher present has no signed reporting agreement. Operator reviewed with employee and signed at time of inspection.
