Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the ice shroud is scored and has a stain. This was cleaned, the stain remained.
Can opener blade with rust that has pitted the surface. Observed : rust on the can opener blade.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed : scoop handle touching dry seasonings, lower shelf on the prep table. A gloved hand was used to make the adjustment.

No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no Employee Reporting forms, and emailed a blank copy. No current employee training.
Required employee training expired for all employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed all certificates are expired.
Open dumpster lid. Observed the dumpster lid was open. This was closed during the inspection.

No measuring device available for measuring utensil surface temperature when using hot water as sanitizer in a dishmachine. Observed the hi temp dish wash machine used in the kitchen, does not have an irreversible temperature taking device for surface temperatures.

Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed: less than 100 ppm for quaternary amonia. Sanitizer was added to 200 ppm.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed: the shroud in the ice maker is soiled. ALSO, the can opener blade had dried food debris on it. These were all cleaned during the inspection. ALSO, the microwave ceiling had moist debris on it.
