Reach-in cooler across from the fryer: 52°F ambient temperature in the top left section, and in both the top and bottom sections of the right side. The operator checked the cooler's temperature at 10:30 a.m. today. (onions and peppers (48F - Cold Holding); hard boiled eggs (46F - Cold Holding); coleslaw (44F - Cold Holding); sliced tomatoes (47F - Cold Holding); blue cheese crumbles (47F - Cold Holding); raw burger (45F - Cold Holding); Dairy (45F - Cold Holding)
3 fly sticky tape hanging in between dish washer area and food preparation area. All 3 fly sticky tape have dead flys on them. Operator removed fly sticky tape.

Employee with no beard guard/restraint while engaging in food preparation. Employee put on a beard guard.

Items had been in cooler since 11 AM. Operator moved to walk in cooler for rapid cooling.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Chlorine sanitizing dishmachine in bar at 0 ppm. Operator primed machine and new reading of dishmachine is at 100 ppm.

Observed a total of 4 flies in establishment. 2 flies near warewashing area at the end of the cook line, 1 fly in wait station area near ice machine, and 1 fly near battering station off the cook line. Observed back door in area propped open. Discussed importance of door remaining shut to eliminate an entry point for pest. Discussed potential solutions with operator.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Coleslaw (50F - Cold Holding) in wait station area cooler. Operator stated items had been in cooler for less than 4 hours. Operator moved to walk-in cooler for rapid cooling. Operator replaced item and placed all items on an ice bath.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed Queso (105F - Hot Holding) in steam table to the left of the cook line and had been in steam table for less than 4 hours. Employee started to reheat in microwave.

Handwash sink not accessible for employee use at all times. Handwash sink off cook line next to battering station to the right of the fryers inaccessible. Pipe disconnected to sink and water dumps on to the floor. Operator stated they had called plumber for repair.
