Raw animal food stored over/not properly separated from ready-to-eat food. In reach in cooler across from 3 compartment sink, pan of raw stuffed crab shells stored over ready to eat coleslaw. Operator moved crab shells to bottom shelf.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In stand up warming unit on cook line, ribs (116F - Hot Holding); chicken (115F - Hot Holding). Cook stated food was not fully wrapped and door was opened and shut all morning. Cook wrapped up food and put temperature up on unit.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink cup and bag of candy on top of lid in steam table on cook line. Cook discarded cup and moved bag of candy.
Equipment in poor repair. Handles missing off two microwaves on cook line.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. On cook line in steam table, collard greens (126F - Hot Holding). Manager stated they had been in warmer for approximately 30 minutes. Manager removed from warmer and reheated collard greens to 165F.
Toxic substance/chemical improperly stored. On cook line, on bottom shelf of steam table spray bottle of disinfectant stored next to go bowls. Manager removed spray bottle from shelf and put in back room.
Equipment in poor repair. Multiple gaskets on cook line torn. Manager stated they have ordered new ones and are waiting for them to arrive.
Ice buildup in reach-in freezer and/or walk-in freezer. In reach in freezer on cook line across from triple sink, ice buildup up present.

Equipment in poor repair. In all standing reach in cooler, torn gaskets. Operator stated he has ordered new gaskets and are awaiting technician to instill them.
Floor, walls and or ceilings soiled/has accumulation of debris. Floor under warming unit has build up. Walls by triple sink is soiled.

Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee using personal cell phone on cook line, placed gloves on, and immediately began preparing food order without washing hands. Manager coached employee on proper hand washing procedures. Employee removed gloves and washed hands.

Wet wiping cloth not stored in sanitizing solution between uses. Wiping cloth stored in empty sanitation bucket in dining area, manager made sanitizer at 200ppm, quaternary ammonium.
